External Case Manager/Marketing Position For sub-acute/long term care setting Full Time with Most Week-ends off. Excellent Salary and Benefits Package.
Friendly, outgoing, detailed oriented Nurse to coordinate the admissions from area hospitals.
Strong Knowledge of All Insurances to include but not limited to Medicare, Medicaid, Managed Care Preferred but will train the right canidate.
Education/Personal Requiements:
Minimum two Year Experience in Long Term Care/Rehabiliation and or Acute Care Sevices
Excellent Nursing Assessment Skills
Must be Self Directed
Knowledge of Basic Computer and Media Skills Required

Summary:
External Case Manager will assess and monitor an individual's status to determine appropriateness for continued medial services and will coordinate and evaluate continued services through communication and collaborating with individual family and other health care professionals.
Insurance verification on all perspective admissions. Collect data regarding evaluation and conversion to admissions. Identify cases appropriate for referred setting. Evaluate and make recommendations to facility or alternate level of care. Promote facility in the community by interacting with various organizations, physicians, hospitals and health care professionals. Maintain the patient's confidentiality and privacy so that they are treated with dignity and respect at all times. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment.
Position will be responsible for but not limited to:
Cultivating referral sources for maximum census building though community interaction
Promote facility in the community by interacting with various organizations, physicians, hospital case mangers and health care professionals.
Identify clients appropriate for referred setting by reviewing medical records and financial information.
Interacting daily with area Hospital case managers
Interacting with third party insurance organizations to verify coverage as well as build referral relationships
Achieving census goals through community based sales techniques
Maintaining and expanding referral leads with frequent contact and follow up
Coordinating and conduct onsite and offsite events
Current Nursing License, RN/LPN issued by the NJ State Board of Nursing required.
Experience in Marketing to build Census in Long Term Care/ Rehabilitative Services Setting preferred.
Pleasant, outgoing personality with good organizational and communication skills a must.
Estimated Salary: $20 to $28 per hour based on qualifications.
